Enrollment Requirements
All students attending Brookside Charter School must reside within the Kansas City Public School District Boundaries.
When enrolling, parents/guardians must provide the following information:
- Household Information – address and phone numbers
- Parent Information – work and cell phone numbers, email addresses
- Student Information – demographic, health/medical and academic information
- Emergency Contact – phone numbers
When enrolling, parents/guardians must provide the following documents:
- Two Proofs of Residency – Rental/ Lease Agreement, Deed, Mortgage Statement, KC Water Bill, Spire Bill, Evergy Bill. Bills must be dated within the past 30 days. If you are unable to provide these documents, please reach out to the school registrar at enrollment@brooksidecharter.org.
- Child’s Current Immunization Record
- Parent/Guardian Government Issued ID
- Child’ Birth Certificate
- IEP/504 Documents (if applicable- new students only)
- Student Insurance Card (if applicable)
